Phoenix Magnet Academy is authorized to disclose the information contained in this form and otherwise collected by or on behalf of Phoenix Magnet Academy:

The collection, use and disclosure of such personal information is authorized for educational consultants, agents, immigration consultants and other service providers of Phoenix Magnet Academy.
Photographs, video, and work samples of students may be used by Phoenix Magnet Academy in the newsletter, website, social media and in other promotional material.
Phoenix Magnet Academy will not sell personal information belonging to website visitors, users, and other consumers.
